Job summary

North Country Healthcare, Inc. is seeking a Hospice Nurse Practitioner (APRN) to deliver comprehensive palliative and hospice care in New Hampshire. You will work with the Hospice Medical Director and an interdisciplinary team to assess patients, manage symptoms, and adjust treatment plans.

This role emphasizes education and support for patients and families in alignment with CMS requirements. The APRN will promote high-quality end-of-life care, participate in advance care planning, and ensure

Qualifications

Must hold a Master’s or Doctoral Degree in Nursing.
Must be licensed as an APRN in New Hampshire.
Nurse Practitioner Certification required; Hospice/ Palliative preferred.

Responsibilities

Provide comprehensive hospice and palliative care to patients with terminal illnesses.
Collaborate with the Hospice Medical Director and interdisciplinary team.
Develop and revise individualized care plans and goals of care.
Educate patients and families on disease progression, medications, and advance directives.
Conduct face-to-face recertifications as per CMS requirements.
Ensure documentation meets CMS CPT and HIPAA standards.
