Ennoble Care is conducting physician-supported hospice services in the Atlanta area. We seek an experienced Per Diem Nurse Practitioner to provide face-to-face assessments for hospice patients, applying medical techniques under supervision while caring for a diverse group with chronic diseases.

Responsibilities include scheduling reviews, confirming F2F encounters, evaluating eligibility, medication reconciliation, symptom management, and clear communication with the interdisciplinary team to

Responsabilités

• Review daily schedule and visit locations
• Confirm all required Face-to-Face (F2F) encounters within regulatory timeframe
• Review patient hospice eligibility, terminal diagnosis, and prior certifications
• Review nursing notes, IDG notes, hospitalizations/ER visits, and medication changes
• Review patient history and prior F2F documentation; identify LCD eligibility indicators
• Educate patient/family on disease progression and expectations
• Reinforce goals of care and hospice philosophy
• Coordinate with Case manager, Medical director, and update clinical leadership on high-risk patients
• Document continued decline with PPS score, infections, dependence, and prognosis indicators
