Frontier Hospice is currently hiring Per Diem Hospice Nurse Practitioners to deliver compassionate patient care in and around the Edmond area. You will evaluate health histories, diagnose conditions, and develop personalized care plans in collaboration with patients, families, and the hospice team.

Formación

• Master's degree required; hospice or palliative care experience preferred
• Current unencumbered RN license and NP license in state of practice
• Certification in hospice/palliative nursing or related specialty preferred
• Excellent verbal and written communication abilities
• Computer proficiency with EMR systems
• CPR certification required by state (if applicable)
• Valid driver's license, vehicle insurance, and reliable transportation

Responsabilidades

• Deliver compassionate medical care, palliate, and manage terminal illnesses under physician supervision
• Collaborate with attending physician, hospice medical director, and other practitioners to assess eligibility for hospice care
• Coordinate and oversee patient care with the interdisciplinary team
• Educate and counsel patients and families on care goals, preventive care, and psychosocial needs
• Provide input on initial certification and recertification for hospice care
• Maintain up-to-date knowledge of Medicare/Medicaid, state and federal hospice regulations
