Frontier Hospice is currently hiring Per Diem dedicated Hospice Nurse Practitioners to deliver exceptional patient care in and around the Edmond Area. You will evaluate health histories, diagnose, and manage health problems by developing personalized care plans in collaboration with patients, families, and the hospice team.

Formación

• Master's degree with at least 1 year hospice/palliative care experience as an RN or NP preferred.
• Current unencumbered license in the state of practice as RN and NP.
• CHPN or similar certification preferred.

Responsabilidades

• Deliver hospice medical care under the supervision of a collaborating physician.
• Collaborate with hospice physicians and interdisciplinary team on patient care.
• Coordinate and oversee patient care in the hospice setting.
• Educate patients and families on care plans and goals.
• Provide input on initial certification and recertification decisions.
• Stay compliant with Medicare/Medicaid and state/federal rules.
