Job summary
Continuum Hospice is currently hiring Per Diem dedicated Hospice Nurse Practitioners to deliver exceptional patient care in and around the Barnstable Area. As a Hospice Nurse Practitioner, you will evaluate health histories, diagnose medical conditions, and manage care plans in collaboration with patients, families, and the interdisciplinary team.

Qualifications
- Master's degree in nursing or related field.
- Current unencumbered RN and NP licenses in the state of practice.
- CHPN or similar hospice/palliative certification preferred.
- Excellent verbal and written communication skills.
- Proficient with computers and EMR systems.
Responsibilities
- Deliver comprehensive hospice medical care and palliate terminal illnesses under physician supervision.
- Collaborate with attending physicians and hospice medical team to assess and manage patient needs.
- Coordinate patient care with interdisciplinary team for holistic management.
- Educate patients and families on care plans, prevention, and psychosocial/spiritual needs.
- Provide input on initial certification and recertification for hospice eligibility.
- Maintain up-to-date knowledge of Medicare/Medicaid and state/federal hospice regulations.
