Job summary

Dover Health seeks a Hospice Nurse Practitioner to deliver compassionate, patient-centered hospice care to individuals with terminal illnesses. You will perform face-to-face visits for eligibility, recertification, symptom management, and collaborate with an interdisciplinary team to ensure high-quality care.

You will educate patients and families, document in EMRs, and travel to homes or facilities as needed to support ongoing care and compliance with regulations.

Qualifications

• Master’s degree in Nursing and completion of an accredited Nurse Practitioner program.
• Current RN and NP licensure in the state of practice; national NP certification (ANCC or AANP).
• Minimum of 1–2 years of clinical experience, preferably in hospice, palliative care, geriatrics, oncology, or home health.
• Knowledge of end-of-life care, pain management, and palliative care principles.
• Strong communication, interpersonal, and emotional resilience skills to support patients and families.
• Ability to use electronic medical records and maintain compliance with healthcare regulations.
• CPR certification and, if applicable, controlled substance agreement

Responsibilities

• Conduct face-to-face assessments for hospice eligibility and recertification requirements.
• Develop and implement individualized care plans addressing physical, emotional, and spiritual needs.
• Manage pain and symptoms, including prescribing medications and adjusting treatments as needed.
• Collaborate with physicians, nurses, social workers, chaplains, and other healthcare professionals to provide comprehensive care.
• Provide emotional support and education to patients and families regarding disease progression, prognosis, and end-of-life decisions.
• Document patient care accurately in electronic medical records and ensure compliance with hospice regulations and Medicare standards.
• Serve as a clinical resource for staff and the community regarding hospice eligibility, levels of care, and symptom management.
• Travel to patient homes, hospice facilities, or long-term care centers as required.
• Other duties as assigned.
