Job summary

Frontier Hospice in the Lawton area is seeking per diem Hospice Nurse Practitioners to deliver specialized end-of-life care. You will evaluate health histories, diagnose conditions, and develop personalized care plans while collaborating with patients, families, and the care team.

Responsibilities include coordinating care with the hospice interdisciplinary team, educating families, and ensuring compliance with state Nurse Practice Act. Per diem visits are remunerated.

Qualifications

• Master's degree in nursing or a related field.
• Current RN and NP licenses as applicable in the practicing state.
• CPR certification is preferred or required by state laws.
• Excellent verbal and written communication abilities.
• Proficient in typing and using EMR and office software.

Responsibilities

• Deliver comprehensive hospice medical care and palliate for terminal illnesses under physician supervision.
• Consult with attending physician, hospice medical director, and other practitioners to assess eligibility and ongoing care needs.
• Review clinical information and coordinate patient care with the interdisciplinary team.
• Educate patients and families, addressing preventive care, medical issues, and spiritual concerns.
• Provide input to the Hospice Medical Director on eligibility for initial certification and recertification.
• Stay up-to-date with Medicare/Medicaid and state/federal hospice regulations.
