Job summary

Frontier Hospice is actively hiring Per Diem Nurse Practitioners to deliver compassionate hospice care in the Chickasha area. You will evaluate health histories, diagnose conditions, and develop personalized care plans in collaboration with patients, families, and the care team.

Responsibilities include coordinating care with the interdisciplinary team, educating families, and ensuring compliance with state nurse practice acts.

Qualifications

• Master's degree in nursing with preferred hospice or palliative care experience.
• Current unencumbered RN and NP licenses in state of practice.
• Certification in Hospice/Palliative care (CHPN) or geriatrics/pediatrics preferred.

Responsibilities

• Provide comprehensive hospice medical care under physician supervision.
• Collaborate with attending physician and hospice team for holistic patient care.
• Coordinate patient care with interdisciplinary team and families.
• Educate patients and families on care goals and disease progression.
• Support eligibility assessment for initial certification and recertification.
