Job summary

Frontier Hospice is currently hiring Per Diem dedicated Hospice Nurse Practitioners to deliver exceptional patient care in and around the Lawton Area! As a Hospice NP, you will evaluate health histories, diagnose conditions, develop personalized care plans, and collaborate with patients, families and the care team to provide compassionate end-of-life services in the Lawton region.

The role requires a current NP license, hospice experience preferred, and reliable transportation.

Qualifications

Master's degree in nursing with hospice/palliative care experience preferred.
Current unencumbered RN and NP licenses in the practicing state are required.
Hospice certification (CHPN) or related hospice/palliative certification is preferred.
Excellent verbal and written communication skills; ability to articulate complex problems.
Proficient with computers and EMR systems; CPR certification may be required by state.
Must have a valid driver's license, vehicle insurance, and reliable transportation.

Responsibilities

Deliver comprehensive hospice medical care and palliate for terminal illnesses under physician supervision.
Collaborate with attending physician, hospice medical director, and other clinicians to assess eligibility and care needs.
Coordinate patient care with the interdisciplinary team to ensure holistic, patient-centered outcomes.
Educate patients and families, addressing preventive care, medical issues, and psychosocial concerns.
Provide input on patient eligibility for initial certification and recertification for hospice care.
Maintain up-to-date knowledge of Medicare/Medicaid, state, and federal hospice regulations.
