Job summary

AmeriPharma is seeking a dedicated Registered Nurse (RN) with home infusion expertise to join our team. The role blends infusion therapy with patient education, providing in-home care and supportive services for treatments such as TPN, IVIG, and IV biologics.

Under the supervision of the Director of Nursing, you will conduct visits, educate patients, document care, and collaborate with pharmacy teams to ensure seamless therapy and optimal outcomes for patients and families in home settings.

Qualifications

• Graduate of an accredited nursing school with a current RN state license.
• 5+ years of experience in home infusion and infusion centers.
• 3+ years with IV Immunoglobulins and IV Biologics; CRNI certification is preferred.
• Experience with peripheral IV starts, infusion therapy, and phlebotomy for adults and pediatric patients is preferred.
• Effective interpersonal, time management, and organizational skills.
• Strong written and verbal communication skills.
• Organizational ability to maintain consistently accurate records.
• Ability to direct information to the appropriate healthcare provider.
• Proficient in computer skills, including word processing, and efficient use of internet and email.
• Current CPR and BLS certification, valid driver’s license, reliable transportation, and automobile insurance.

Responsibilities

• Conduct in-home infusion therapy visits according to the plan of care.
• Prepare clinical/nursing notes and document patient interventions in records.
• Act as a clinical resource, collaborating with pharmacy departments.
• Maintain knowledge of drug side effects and interactions.
• Offer telephone and in-person patient education on therapy protocols and home infusion procedures.
• Ensure patients understand therapy plans, medications, and adherence.
• Serve as patient liaison between patient, family, and the pharmacy.
• Perform comprehensive head-to-toe assessments and initiate preventative care.
• Use health assessment data to determine patient needs.
• Manage patient and family expectations regarding services and outcomes.
• Establish diagnoses based on assessment and home health care focus.
• Develop care plans with skilled interventions and necessary services.
• Regularly evaluate patient progress and revise care plans.
• Perform other duties as assigned.
