Job summary

Omega Home Health Care Services Inc is seeking a Registered Nurse with home health experience to provide in-home patient care across CA regions, including LA and surrounding counties. You will perform assessments, implement and adjust care plans, administer medications, and educate patients and families while coordinating with the agency.

The role requires CA RN licensure, CPR, valid driver’s license, and 2+ years of home health experience; travel to on-site patient locations is expected.

Qualifications

• California RN license required.
• Two years’ home health/public health experience preferred.
• Current CPR certification required.
• Valid driver’s license and reliable transportation.
• Graduate of NLN-accredited nursing program required.
• Excellent observation and communication skills.
• Ability to perform lab draws and wound care.
• Experience with IVs (PICC line, Central, Midline).
• Self-directed and able to work with minimal supervision.

Responsibilities

• Completes initial patient assessment and develops a care plan.
• Regularly re-evaluates patient nursing needs and revises plans.
• Administers prescribed medications and treatments.
• Provides patient and family education and discharge planning insights.
• Maintains documentation and coordinates with agency physicians.
• Supervises home health aides and coordinates care delivery.
• Ensures equipment and services are arranged as needed.
• Communicates changes in patient status to the agency.
