Job summary

Omega Home Health Care Services Inc. seeks Registered Nurses dedicated to delivering high-quality home health care across Los Angeles County and surrounding areas. This per diem role requires CA RN license, CPR, and the ability to work independently at patients' homes.

You will assess patients, develop and revise care plans, administer treatments, and educate families. Reliable transportation and a valid driver's license are essential for visiting patients in the community.

Qualifications

CA RN license required; current licensure.
Ability to perform lab draws and wound care.
Experience with IVs (PICC, Central, Midline).
Tracheostomy management experience.
CPR certification required.
Must have valid driver’s license and insured vehicle.
Minimum 2 years’ home health/public health preferred.
Self-directed with strong communication and organizational skills.

Responsibilities

Completes an initial assessment of patient and family to determine home care needs.
Initiates the plan of care and revises it as patient status changes.
Administers medications and nursing treatments as prescribed.
Counsels patient and family in meeting nursing needs.
Prepares clinical notes and communicates changes to the agency.
Coordinates care with physicians and community health partners.
Ensures equipment and services are arranged; supervises home health aides every two weeks.
Identifies discharge planning needs and assists with transition.
