Qualifikationen

• California RN license required.
• Ability to perform lab draws.
• Ability to perform wound care.
• Experience with IV therapy (PICCs, Central lines, Midlines).
• Tracheostomy care experience preferred.
• NLN-accredited nursing program graduate required.
• Current CPR certification required.
• Must have a valid driver’s license and reliable transportation.
• Minimum two years in home health or public health preferred.
• Self-directed with good communication and problem-solving skills.
• Strong observation and organizational abilities.

Aufgaben

• Completes an initial assessment of patient and family to determine home care needs.
• Regularly re-evaluates patient nursing needs and revises care plan.
• Initiates the plan of care and updates as patient status changes.
• Develops a care plan with goals based on nursing diagnosis.
• Administers medications and treatments per physician orders.
• Counsels patient and family on nursing needs.
• Provides health instructions per care plan.
• Identifies discharge planning needs and implements them.
• Prepares clinical notes and updates the agency.
• Coordinates with physician and community health personnel.
• Assists in arranging equipment and services; oversees home health aides.
