Job summary

Best Home Health Providers Inc. is hiring a per diem Physical Therapist to deliver skilled home-based care across our Bay Area network. You will assess, treat, and educate patients to regain mobility and independence, while coordinating with physicians and caregivers.

Qualifications include a CA PT license, 2+ years of PT experience, CPR (AHA), a valid driver's license, and a flexible schedule with potential to convert to full-time based on performance.

Qualifications

• PT degree from an accredited program.
• Active California Physical Therapy license.
• Minimum 2 years of PT experience; home health background is a plus.
• CPR certification (AHA).
• Valid driver’s license and insured vehicle.
• Flexible schedule and potential to convert to full-time.

Responsibilities

• Evaluate patients’ functional status and develop treatment plans.
• Deliver PT services per physician's plan and CA PT Act.
• Educate patients and families on exercises and recovery goals.
• Collaborate with physicians and care team; report changes.
• Supervise PT Assistants and oversee Home Health Aide care bi-weekly.
• Manage documentation including progress summaries and discharge planning.
• Identify needs for additional home health services and coordinate referrals.
• Perform other duties as required for patient services.
