Job summary

Best Home Health Providers Inc. is seeking a per diem Physical Therapist to deliver skilled home health care across the Bay Area and beyond. You will assess patients, develop individualized plans, and coordinate with physicians and caregivers to support safe, effective recovery at home.

Applicants should have a PT degree, CA license, CPR certification, and at least 2 years of PT experience, with home health background preferred. A reliable vehicle and driver's license are required.

Qualifications

• Must have a Bachelor’s or Master’s degree in Physical Therapy from an accredited program.
• Active California Physical Therapy license.
• Minimum 2 years of PT experience; home health background preferred.
• Strong communication skills and the ability to connect with patients and families.
• Current CPR certification with AHA.

Responsibilities

• Evaluate patients' functional status, mobility, muscle strength, gait, and ROM to develop individualized treatment plans.
• Deliver physical therapy services in accordance with the physician's plan of care and California's Physical Therapy Practice Act.
• Educate patients and families on therapeutic exercises, equipment use, and recovery goals.
• Collaborate with physicians and the care team—reporting changes in patient condition and participating in case conferences.
• Supervise PT Assistants and oversee Home Health Aide care bi‑weekly when PT is the sole skilled service.
• Manage documentation including clinical progress summaries and discharge planning.
• Identify needs for additional home health services and coordinate referrals as appropriate.
• Perform other duties as required for patient services
