Best Home Health Providers is seeking a per diem Physical Therapist in California to deliver skilled home health services. You will evaluate mobility, design treatment plans, and educate patients and families to support independence and recovery.

Responsibilities include working with physicians, supervising aides when needed, and ensuring accurate documentation. A CA PT license, CPR, and 2+ years of PT experience are required.

Qualificações

• Bachelor’s or Master’s degree in Physical Therapy from an accredited program.
• Active California Physical Therapy license.
• Minimum 2 years of PT experience; home health background preferred.
• Current CPR certification with AHA.

Responsabilidades

• Evaluate patients’ functional status, mobility, strength, gait, and ROM to create individualized plans.
• Deliver PT services per physician plan and CA PT Practice Act.
• Educate patients and families on exercises, equipment, and recovery goals.
• Collaborate with physicians and care team; report changes and participate in case conferences.
