Job summary

Best Home Health Providers, Inc. is seeking a per diem Physical Therapist to deliver skilled care in patients' homes across the Bay Area. You will evaluate mobility, design plans of care, and educate patients and families to promote recovery and independence.

Requirements include a Doctoral or Master’s in Physical Therapy, active CA license, CPR (AHA), and at least 2 years of PT experience (home health a plus). A vehicle and valid driver’s license are needed for patient visits.

Qualifications

• Bachelor’s or Master’s degree in Physical Therapy from an accredited program.
• Active California Physical Therapy license.
• Minimum 2 years of PT experience; home health background is a plus.
• Strong communication skills and ability to connect with patients and families.
• Current CPR certification with AHA.

Responsibilities

• Evaluate patients’ functional status, mobility, muscle strength, gait, and ROM to develop individualized treatment plans.
• Deliver physical therapy services in accordance with physician’s plan of care and California’s Physical Therapy Practice Act.
• Educate patients and families or caregivers on exercises, equipment use, and recovery goals.
• Collaborate with physicians and the care team—reporting changes in patient condition and participating in case conferences.
• Supervise PT Assistants and oversee Home Health Aide care bi-weekly when PT is the sole skilled service.
• Manage documentation including progress summaries and discharge planning.
• Identify needs for additional home health services and coordinate referrals as appropriate.
• Perform other duties as required for patient services.
