Job summary

Best Home Health Providers Inc. seeks a per diem Physical Therapist to evaluate mobility, strength, gait, and ROM, and deliver skilled PT in patients’ homes across the Bay Area. You will educate patients and families, collaborate with physicians, and supervise assistants as needed.

Minimum 2 years PT experience, CA license, and CPR certification required. Flexible schedule, mileage reimbursement, and referral bonuses are among offered benefits.

Qualifications

• PT licensed in California
• Minimum 2 years PT experience; home health preferred
• Able to work per diem and travel within Bay Area

Responsibilities

• Evaluate patients’ functional status, mobility, strength, gait, and ROM to tailor plans.
• Deliver PT services per physician’s plan and CA PT Act.
• Educate patients and caregivers on exercises and equipment.
• Collaborate with physicians; report changes and participate in case conferences.
• Oversee PT Assistants and Home Health Aide when PT is sole skilled service.
• Maintain documentation including progress notes and discharge planning.
• Identify needs for additional home health services and coordinate referrals.
• Perform other duties as required for patient services.
