Job summary

Best Home Health Providers, Inc. is seeking a Physical Therapist to provide skilled services in home health settings across the Bay Area. The role emphasizes mobility, independence, and patient confidence as part of a compassionate, family‑oriented team.

Per diem compensation ranges $180–$200 per visit. California PT licensure, CPR, and a valid driver’s license are required; home health experience is a plus. Flexible scheduling and potential path to full‑time based on performance are offered.

Qualifications

• Bachelor’s or Master’s degree in Physical Therapy from an accred­ited program
• Active California Physical Therapy license
• Minimum 2 years of PT experience; home health background is a plus
• Strong communication skills and the ability to connect with patients and families
• Current CPR certification with AHA
• Valid driver’s license and reliable, insured vehicle

Responsibilities

• Evaluate patients' functional status, mobility, muscle strength, gait, and ROM to develop individualized treatment plans
• Deliver physical therapy services per physician’s plan of care and California PT Practice Act
• Educate patients and families on exercises, equipment use, and recovery goals
• Collaborate with physicians and care team, report changes, participate in case conferences
• Supervise PTAs and oversee Home Health Aide care when PT is sole skilled service bi-weekly
• Manage documentation including progress summaries and discharge planning
• Identify needs for additional home health services and coordinate referrals
• Perform other duties as required for patient services
