Job summary

Premier Home Health Providers Inc. is seeking a per diem Occupational Therapist to deliver home-based therapy across California. You will assess patients in their home, develop personalized treatment plans, and guide clients in daily living activities.

You will collaborate with physicians, caregivers, and the broader care team to ensure safe, effective care. The role offers mileage reimbursement, flexible scheduling, and potential transition to full-time based on performance.

Qualifications

• Licensed Occupational Therapist in California.
• Active California OT license.
• Minimum 2 years of OT clinical experience.
• Current CPR certification.
• Valid driver’s license and insured vehicle for travel to patient homes.

Responsibilities

• Evaluate patients’ functional abilities and home environments for safety.
• Develop and communicate personalized OT treatment plans.
• Teach daily living skills to patients post-illness or injury.
• Instruct on adaptive equipment and orthotic devices.
• Maintain accurate clinical records and report changes to the care team.
• Coordinate with the care team for seamless patient-centered care.
• Educate families and caregivers on treatment approach.
• Supervise OT Assistants and oversee home health aide care when OT is the sole skilled service.
• Support discharge planning as patients transition home.
