Job summary

Premier Home Health Providers Inc. seeks an Occupational Therapist to deliver per diem home-based care across California, helping patients regain independence in the comfort of their own homes.

You will evaluate functional abilities, design personalized treatment plans, teach daily living activities, and coordinate with physicians and the care team. California OT license, NBCOT, CPR, and a minimum of 2 years of OT experience are required, with documentation and family education responsibilities.

Qualifications

• Graduate of an OT curriculum accredited by AOTA.
• Registered with NBCOT.
• Active California OT license.
• Minimum 2 years of clinical OT experience.
• Current CPR certification (AHA).

Responsibilities

• Evaluate patients' functional abilities and home safety.
• Develop personalized treatment plans and communicate them to physicians and the care team.
• Teach and retrain patients in daily living skills.
• Recommend and instruct adaptive equipment, splints, and devices.
• Maintain accurate clinical records and report changes in patient condition.
• Coordinate with the broader care team to ensure patient-centered care.
• Educate family members and caregivers on the treatment approach.
