Best Home Health Providers Inc. is seeking an Occupational Therapist to support patients in their homes across the Bay Area and surrounding areas.

The role focuses on helping patients regain independence and safely manage daily activities, enabling a smooth transition from facility to home. This per-diem position offers $130 per visit, with flexible scheduling and the opportunity to work with a compassionate, family-oriented team.

Qualifications

• Must have active California OT license and NBCOT registration.
• Minimum 2 years of clinical OT experience.
• Graduate of an accredited OT curriculum.
• Current CPR certification with AHA and a valid driver’s license with reliable vehicle.

Responsibilities

• Evaluate patients’ functional abilities and home environments for safety.
• Develop and communicate personalized treatment plans.
• Teach and retrain patients in daily living skills.
• Recommend adaptive equipment and devices.
• Maintain accurate clinical records and report changes.
• Coordinate with the care team for patient-centered care.
• Educate family members and caregivers on the treatment approach.
• Supervise OT Assistants and oversee home health aide care when OT is the sole skilled service.
• Support discharge planning and travel to patient homes.
