Job summary

Caring Hand Home Health Solutions, Inc. is seeking an Occupational Therapist to deliver in-home care on a per diem basis in California. You’ll help patients regain independence, improve function, and safely navigate daily life with patient-centered, home-based therapy.

Join a team valuing empathy and high-quality service in a family-oriented atmosphere. Requirements include CA OT licensure, active CPR, NBCOT certification, and 2+ years of OT experience; travel to patient homes is required.

Qualifications

• Graduate of an OT curriculum accredited jointly by AMA and AOTA.
• Registered with NBCOT and licensed in California.
• Active California OT license with CPR certification.
• Minimum 2 years of clinical OT experience.
• Ability to travel to patient homes with a reliable vehicle.

Responsibilities

• Evaluate patients’ functional abilities and home environments to identify needs and safety concerns.
• Develop personalized treatment plans and communicate them to physicians and the care team.
• Teach and retrain patients in daily living skills affected by illness or injury.
• Recommend and instruct patients on adaptive equipment, splints, and prosthetic/orthotic devices.
• Maintain accurate clinical records and promptly report changes in patient condition.
• Coordinate with the broader care team to ensure seamless, patient-centered care.
• Educate family members and caregivers on the patient’s treatment approach.
• Supervise Occupational Therapy Assistants and oversee home health aide care when OT is the sole skilled service.
• Support discharge planning as patients transition out of home health care.
• Perform other duties as required for patient services.
