Job summary

Liberty Home Care in Swan Quarter, North Carolina, is seeking an experienced Occupational Therapist (OT) for Home Health. This per diem, PRN position involves delivering direct patient care, conducting comprehensive evaluations, and establishing a measurable plan of care tailored to each patient’s needs.

You will coordinate with COTA staff, document progress, and collaborate with the Patient Care Coordinator and physician to ensure quality home-based rehabilitation.

Qualifications

• Must have a bachelor's degree in occupational therapy from an accredited program.
• Successful completion of the National Certification Exam and state licensure.
• At least one year of experience in a non-home health environment preferred.

Responsibilities

• Provide patient care services with direct treatment and follow-up.
• Evaluate new patients using motor, sensory-perceptual-cognitive assessments as indicated.
• Develop and implement a plan of care based on assessments.
• Set measurable goals aligned with the patient’s evaluation.
• Document care data thoroughly and timely, showing progress or decline.
• Coordinate with COTAs and supervise COTA patient care monthly.
• Submit daily/weekly visit schedules to Scheduler.
• Communicate with intercompany contacts as needed.
• Discuss reimbursement policies and treatment plans with patients and family.
• Refer to appropriate disciplines with Patient Care Coordinator.
• Provide in-service education and assist in orientation of new staff.

Education

Bachelor’s degree in Occupational Therapy

Current state licensure to practice OT

National Certification Exam completion
