Job summary

Liberty Home Care in Durham, NC is seeking a Per Diem Occupational Therapist to join our home health team. You will provide direct patient care, assess new patients, and develop individualized plans of care tailored to each diagnosis.

The role requires a bachelor’s in occupational therapy, current state licensure, and ongoing continuing education. You will work with COTAs, document progress, and coordinate with the care team to optimize patient outcomes.

Qualifications

• Bachelor’s degree in occupational therapy from an accredited program.
• Successful completion of the National Certification Exam and current NC licensure.
• Prefer a minimum of one year experience in a non-home health setting.
• CPR certification and strong communication/organization skills.
• Minimum of twelve hours of continuing education annually.

Responsibilities

• Provides patient care services with direct treatment and follow-up.
• Evaluates new patients using motor, sensory-perceptual-cognitive, and performance evaluations as indicated.
• Develops a plan of care based on assessment findings.
• Sets measurable goals aligned with the patient’s assessment.
• Documents patient care data accurately and timely.
• Coordinates with COTAs and provides monthly supervision of their patient care.
• Submits daily/weekly patient visit schedules to the Scheduler.
• Communicates with intercompany and intracompany contacts.
• Discusses reimbursement policies and treatment plans with Patient Care Coordinator and physician.
• Collaborates with the Patient Care Coordinator on referrals.
• Provides in-service education and supervision of peers.
• Participates in orientation of new employees.
