Qualifications

• Bachelor's degree in occupational therapy and state licensure.
• National Certification Exam completion and active state licensure.
• CPR certification and strong communication/organization skills.
• Minimum twelve hours of continuing education annually.

Responsibilities

• Provides patient care and direct treatment with follow-up.
• Evaluates new patients using motor, sensory-perceptual, or performance assessments as indicated.
• Develops a plan of care based on assessment findings.
• Establishes measurable goals aligned with the patient’s evaluation.
• Documents care data accurately and timely, showing progress or decline.
• Coordinates with COTAs and supervises their patient care monthly.
• Communicates with Patient Care Coordinator, physician, and family about plans.
• Assists with referrals to appropriate disciplines and educates staff as needed.
