Job summary

Liberty Home Care in Jacksonville, North Carolina, seeks an experienced Occupational Therapist for per diem, PRN flexible home health services in Onslow, Jones, Craven, and Carteret counties. You will evaluate patients, develop plans of care, and coordinate with COTAs and the care team to ensure high-quality in-home therapy.

The role requires a bachelor’s degree in occupational therapy, NC licensure, and the National Certification Exam, with CPR certification and continuing education.

Qualifications

• Bachelor’s degree in occupational therapy from an accredited program.
• National Certification Exam completed and licensure current in the state.
• Minimum one year of therapeutic experience in a non-home health setting.
• CPR certified; strong communication and organization skills.
• At least twelve hours of continuing education annually.

Responsibilities

• Provide patient care services with direct treatment and follow-up.
• Evaluate patients new to service per physician orders using motor/sensory/cognitive and performance evaluations.
• Develop a plan of care aligned to diagnosis from assessment findings.
• Establish measurable goals in line with patient assessment.
• Document patient data accurately and timely showing progress or decline.
• Coordinate with COTAs and supervise their patient care monthly.
• Submit daily/weekly patient visit schedules to Scheduler.
• Discuss reimbursement policies with patient/family and care team.
• Refer to appropriate disciplines as needed; provide education and orientation.
