Job summary

Liberty Health is seeking a Home Care Occupational Therapist (OT) to provide patient-centered home health services in Mount Airy, NC. This per diem/PRN role involves direct treatment, evaluation, and ongoing plan-of-care development for individuals recovering at home.

The OT will collaborate with COTAs, document progress, and participate in interdisciplinary care planning. Qualified candidates hold a OT degree, state licensure, CPR, and CE obligations, with travel as needed.

Qualifications

• Bachelor’s degree in occupational therapy from an accredited school.
• Current licensure to practice OT in the state.
• CPR certification and strong communication/organizational skills.
• Minimum twelve hours of continuing education annually.

Responsibilities

• Provides patient care services along with direct treatment and follow-up.
• Evaluates new patients to service per physician orders using motor, sensory-perceptual-cognitive, and performance evaluations.
• Develops a care plan based on assessment findings.
• Establishes measurable goals aligned with the patient’s assessment.
• Documents patient care data accurately and timely, reflecting progress or decline.
• Coordinates with COTAs and provides monthly supervision of COTA’s patient care.
• Submits daily/weekly patient visit schedules to the Scheduler.
• Discusses reimbursement policies with patient/family and treatment plans with the Patient Care Coordinator, physician, and patient family.
• Coordinates referrals to other disciplines as needed.
• Provides in-service education and supervises peers and trainees as required.
• Conducts research and assists with orientation of new employees.
