Responsibilities

• Initiates patient care by assessing functional status after referral.
• Formulates a plan of care with measurable goals.
• Implements plan of care per Hand Therapy modalities and safety guidelines.
• Reassesses progress and documents response to treatment.
• Participates in discharge planning and continuity of care.
• Educates patients and develops materials to reinforce treatment.
• Maintains complete evaluations, progress notes, discharge summaries and daily treatment charge sheets.
• Supervises Rehab Aide as indicated and participates in staff meetings and education.
• Contributes to new program development within Rehabilitation Services.
