Job summary

Beth Israel Lahey Health seeks a per diem Occupational Therapist to plan and direct home health services in Massachusetts. The role emphasizes community-based care, patient and family involvement, and coordination with the care team to meet individual goals.

The OT will assess, plan, implement, and revise care plans, supervise home health aides, and ensure adherence to regulations and payer guidelines while maintaining patient satisfaction and outcomes.

Qualifications

• Graduate of an accredited program in Occupational Therapy.
• Licensed, certified Occupational Therapist with current licensure to practice in Massachusetts
• Active BLS (American Heart Association)
• Minimum 2 years' experience preferred
• OASIS experience preferred

Responsibilities

• Plan and direct home care services for patients in their homes and communities.
• Coordinate patient care with COTA staff and supervise delegated visits.
• Complete initial patient assessment and develop patient-specific plan of care.
• Initiate and revise plan of care as patient needs change.
• Instruct and supervise home health aides and coordinate inter-disciplinary care.
