Qualifications

• Ability to prioritize multiple high-risk patients and work with interdisciplinary teams.

Responsibilities

• Intervenes with patients and families for psychosocial needs and supports coping with illness.
• Provides crisis intervention and coordinates discharge planning across care teams.
• Serves as liaison between patients and community resources and referrals.
• Assesses psychosocial and environmental needs related to admission, treatment and discharge.
• Maintains strong organization and time-management to manage multiple tasks.
