Job summary

Baptist Health Bethesda Hospital is seeking a social work case manager to intervene with patients and families for psychosocial needs, provide discharge planning, education, counseling, advocacy, and referrals to address emotional, social, and financial aspects of care.

The role includes crisis intervention, inpatient emotional support, bereavement services, and addressing abuse concerns across neonatal to adult patient groups.

Qualifications

• Master’s degree (MSW preferred) or equivalent in Social Work.
• 0–1 year of social work experience in a hospital or related health care setting; internship considered.
• Ability to work independently with sound judgment in a fast-paced environment.
• Excellent interpersonal, communication, negotiation, analytical, data‑management, computer, and organizational skills.
• Current knowledge of discharge planning, crisis intervention, and community services for insured and uninsured patients.

Responsibilities

• Intervenes with patients and families for psychosocial needs, providing consultation, discharge planning, education, counseling, advocacy, and referrals.
• Addresses emotional, social, and financial aspects of care and facilitates crisis intervention and bereavement services.
• Supports neonatal, infant, pediatric, adolescent, and adult patient groups; collaborates with care teams.
