Bay Cove Human Services is seeking a Case Manager - Per Diem to join a multidisciplinary team providing support, counseling, and community referrals to individuals with substance use and co-occurring disorders. Qualificações

• High School diploma or equivalent required.
• Bachelor's Degree in social work or related field or equivalent experience required.
• Minimum of one year experience in Behavioral Health or Human Services.
• Knowledge of health care, detoxification, addiction, co-occurring disorders and DSM terminology preferred.
• Ability to communicate clearly and persuasively in various situations.

Responsabilidades

• Orient patients to unit rights and guidelines and obtain informed consent.
• Assist clinical team in psychosocial assessments and treatment planning.
• Coordinate services with external providers and assist with appointments.
• Develop aftercare plans and coordinate transition to aftercare services.
• Provide individual and group education and recovery support during detox and withdrawal.
