Job summary

Qualifications

• Bachelor of Science in Nursing required.
• Current California RN license.
• American Heart Association BLS certification.
• 3–5 years of acute care hospital experience.
• 1–2 years of experience as a Case Manager.
• Excellent written and verbal communication.
• Proficient typing and Microsoft Office skills.

Responsibilities

• Assess treatment needs and develop, monitor, and evaluate care plans.
• Coordinate interdisciplinary care to ensure safety, timeliness, and quality.
• Facilitate patient-centered care and monitor progress.
• Maintain effective working relationships across the Health System.
