Job summary

Hope Hospice in Pleasanton, CA seeks an Admissions/Team Nurse to support Nurse Case Managers, cover admissions for eligible hospice patients, and coordinate with the Team Manager.

This per diem position offers 8:30 am–5:00 pm coverage, two days a week or six shifts monthly, with responsibilities including in-person visits, nursing process, and coordination of services with the transdisciplinary team.

Qualifications

• RN license in the State of California (CA) required or active.
• ASN or BSN in nursing from an accredited program.
• Experience in acute care/home health and/or hospice setting preferred.
• Public Health Nursing certification desirable; HPNA certification preferred.

Responsibilities

• Make in-person nursing visits to patients as assigned or as indicated when case managing.
• Use the nursing process to assess, plan, intervene and evaluate patient/family needs and goals.
• Provide effective management of symptoms with escalation to providers as needed.
• Coordinate patient/family services and prioritize needs with transdisciplinary team or outside providers.
• Make telephone contact and/or conduct assessment visits to inform patients about services and assess status.

Admissions/Team Nurse provides coverage for Nurse Case Managers to cover patient/staffing needs and processes admissions for eligible hospice patients in coordination with Team Manager.

Schedule Per diem; 2 days a week or 6 shifts per month; 8:30am - 5:00pm

Office Location Pleasanton, CA

Location Alameda & Contra Costa Counties with occasional coverage to San Mateo County as needed

Hope Hospice follows all CDPH vaccine requirements for healthcare personnel.

Essential Duties & Responsibilities include (but are not limited to):

• Make in-person nursing visits to patients as assigned, or as indicated when case managing.
• Use the nursing process to assess, plan, intervene and evaluate following patient/family needs and goals.
• Provide effective management of symptoms, including prompt escalation to providers or others as indicated.
• Coordinate patient/family services and prioritization of needs with the members of the transdisciplinary team or outside providers/services.
• Make telephone contact and/or conduct assessment visits with potential hospice patients to provide information on agency services and assess patient status.

Education/Experience

• ASN or Bachelor's degree in nursing from an accredited School of Nursing.
• Experience in acute care/ home health and/or hospice setting preferred.

Certificates, Licenses, Registrations

• Current license as a registered professional nurse in the State of California.
• Public Health Nursing certification highly desirable.
• HPNA certification preferred.

Pay Range $58.34 - $72.95 hourly
