Hope Hospice in Pleasanton, CA seeks an Admissions/Team Nurse to support Nurse Case Managers, cover admissions for eligible hospice patients, and coordinate with the Team Manager.
This per diem position offers 8:30 am–5:00 pm coverage, two days a week or six shifts monthly, with responsibilities including in-person visits, nursing process, and coordination of services with the transdisciplinary team.

Admissions/Team Nurse provides coverage for Nurse Case Managers to cover patient/staffing needs and processes admissions for eligible hospice patients in coordination with Team Manager.

Office Location Pleasanton, CA
Location Alameda & Contra Costa Counties with occasional coverage to San Mateo County as needed
