Job summary

Hope Hospice in Pleasanton, CA is seeking a per diem After-Hours Nurse to provide skilled nursing and hospice care to maximize patient comfort and health. The position operates Saturdays and Sundays, 8:30am - 5:00pm, serving Alameda, Contra Costa & San Mateo Counties.

The ideal candidate holds an RN license in California with 1 year of nursing experience and prefers experience in hospice or home health settings. Public Health Nursing certification is desirable; HPNA certification is preferred.

Qualifications

• One year of professional nursing experience in the past three years or a Bachelor's in Nursing from an accredited school.
• Current California RN license; Public Health Nursing certification desirable.

Responsibilities

• Provide continuing care to patients and families per agency policies and regulations.
• Under supervision of After-Hours Nurse, assess, plan, implement, and evaluate nursing services.
• Communicate with attending physicians and community resources to coordinate patient care.
• Maintain regular communication with Team Leaders regarding patient status and issues.

Hope Hospice follows all CDPH vaccine requirements for healthcare personnel.

Essential Duties & Responsibilities include

• Provide continuing care to patients/families as requested and in compliance with agency policies and procedures and applicable federal and state regulations.
• Under the guidance of the After‑Hours Nurse, responsible for patients/families including assessing, planning, implementing, and evaluating nursing services consistent with the plan of care.
• Communicate with attending physicians and other community resources to report changes and coordinate optimal care and use of resources for the patient/family.
• Maintain regular communication with Team Leaders regarding patient/family status and unusual or potentially problematic patient/family issues.

Education/Experience

• One year experience as a professional nurse in the past three years or Bachelor’s degree in Nursing from an accredited School of Nursing.
• Experience in acute care/home health and/or hospice setting preferred.

Certificates, Licenses, Registrations

• Current license as a registered professional nurse in the State of California.
• Public Health Nursing certification highly desirable.
• HPNA certification preferred.

Pay Range

$52.90 - $66.20 hourly plus $2.00 weekend differential
