Hope Hospice in Pleasanton, CA is seeking a Social Worker to join the hospice transdisciplinary team. You will provide supportive services to meet patient and family social, emotional, and financial needs related to terminal illness, loss, and grief.
Schedule: Monday–Friday, 8:30am–5:00pm; Service location in Contra Costa & Alameda Counties. Masters of Social Work (CSWE accredited) and at least 1 year in acute health care required; hospice/home health experience preferred.
