Job summary

Heart of Hope Asian American Hospice Care in San Jose, California seeks a Registered Nurse to perform skilled nursing, assessments, and care planning for hospice patients and families.

The RN will assess and manage medical needs, administer prescribed treatments, and provide compassionate support to patients and families, working in a diverse community with a strong commitment to end-of-life care.

Qualifications

• Minimum 1 year of nursing experience within the last 3 years, or a BSN in a NLN-accredited program.
• Current California nursing license.
• Ability to work independently with strong observation, verbal and written communication skills.
• Cultural sensitivity and ability to cope with emotional stress.
• Experience in hospice/homecare is a plus.
• Bilingual Chinese/English welcomed but not required.
• Covid-19 healthcare personnel compliance.

High-level Job Description

The Registered Nurse (RN) provides skilled nursing and comprehensive assessment in the development of the hospice patient and family plan of care. Skilled nursing services include but not limited to patient assessment, evaluation, and case management of medical needs of the patients; the performance of prescribed medical treatment for pain and symptom control; and the provision of emotional support to both the patients and their families.

Pay Rate And Benefits

• Hourly rate of $50-55
• $250 per patient admission visit
• $150 per regular visit
• Weekday evening on call $150
• Weekend on call $250
• Paid Sick Leave
• 401K retirement plan with optional company matching

Qualifications/Requirements

• Minimum of 1 year of experience as a professional nurse within the last 3 years; or a baccalaureate degree in nursing from a program accredited by the National League for Nursing.
• Current licensure in the State of California.
• Work independently and have excellent observation, verbal and written communication skills, and nursing skills per competency checklist.
• Able to cope with emotional stress tolerant of different lifestyles, cultures, beliefs, and values.
• Knowledge of and commitment to hospice philosophy of care.
• Experience in hospice and homecare are advantages.
• Bilingual Chinese/English Speaking welcomed but not required.
• Compliant with Covid-19 healthcare personnel requirement in all City, County, State and Federal level.
