Job summary

Hope Hospice is seeking a Hospice Nurse Case Manager to identify patient/family needs and provide skilled nursing assessment, planning, and supportive care in accordance with the attending physician’s plan of treatment.

The role follows Monday–Friday, 8:30am–5:00pm with a caseload-based focus in Contra Costa County and parts of Alameda County. California RN license and hospice experience are preferred.

Qualifications

• RN license in California.
• Bachelor's degree in Nursing or one year nursing experience within the last three years.
• Experience in acute care/home health and/or hospice preferred.
• Public Health Nursing certification desirable.
• HPNA certification preferred.

Responsibilities

• Identify patient/family needs and provide skilled nursing assessment, planning, and supportive care.
• Coordinate care with attending physicians, hospice staff, and other agencies as needed.
• Maintain caseload and productivity per agency protocol.
• Communicate regularly with Director of Patient Services and attending physician regarding caseload and patient care.

We are seeking a Hospice Nurse Case Manager to be responsible for identifying patient/family needs and for providing skilled nursing assessment, planning and supportive care and appropriate services in accordance with the attending physician’s plan of treatment and orders.

Monday - Friday; 8:30am - 5:00pm

Location: Contra Costa County & some areas of Alameda County

Hope Hospice follows all CDPH vaccine requirements for healthcare personnel.

Essential Duties & Responsibilities include

• Assume primary responsibility for a patient/family caseload that includes the assessing, planning, implementing and evaluating phases of the nursing process. Maintain a caseload and productivity per agency’s protocol.
• Initiate communication with attending physicians, other hospice staff members and other agencies as needed to coordinate optimal care and use of resources for the patient/family.
• Maintain regular communication with Director of Patient Services/designee to review caseload.
• Maintain regular communication with the attending physician concerning patient/family care needs and response to interventions.

Education/Experience

• One year experience as a professional nurse in the past three years or Bachelor’s degree in Nursing from an accredited School of Nursing.
• Experience in acute care/ home health and/or hospice setting preferred.

Certificates, Licenses, Registrations

• Current license as a registered professional nurse in the State of California.
• Public Health Nursing certification highly desirable.
• HPNA certification preferred.
• Current California Driver’s License and automobile insurance that meets State of California requirements, if driving a car.

Pay Range

$58.90 - $73.65 hourly
