Job summary

Mission Hospice & Home Care in San Mateo, CA is seeking an experienced registered nurse to join our after-hours team. You will provide continuing care, coordinate with physicians, and support patients and families in line with our care plan.

This role requires California RN licensure and experience in home health or hospice; weekend availability is essential. Pay is $52.90–$66.20 per hour with a weekend differential, and we offer a supportive, inclusive work environment.

Qualifications

• One year of professional nursing experience within the past three years or a BSN from an accredited School of Nursing.

Responsibilities

• Provide continuing care to patients/families in accordance with agency policies and regulations.

• Maintain regular communication with Team Leaders regarding patient/family status and issues.

Every Saturday & Sunday; 4:30pm - 12:30am

Mission Hospice & Home Care follows all CDPH vaccine requirements for healthcare personnel.

Essential Duties & Responsibilities include

• Provide continuing care to patients/families as requested and in compliance with agency policies and procedures and applicable federal and state regulations.
• Under the guidance of the After-Hours Nurse, responsible for patients/families including assessing, planning, implementing, and evaluating nursing services consistent with the plan of care.
• Communicate with attending physicians and other community resources to report changes and coordinate optimal care and use of resources for the patient/ family.
• Maintain regular communication with Team Leaders regarding patient/family status and unusual or potentially problematic patient/family issues.

Education/Experience:

• One year experience as a professional nurse in the past three years or Bachelor’s degree in Nursing from an accredited School of Nursing.
• Experience in acute care/ home health and/or hospice setting preferred.

Certificates, Licenses, Registrations:

• Current license as a registered professional nurse in the State of California.
• Public Health Nursing certification highly desirable.
• HPNA certification preferred.

Pay Range:

$52.90 - $66.20 hourly + $2.00 weekend differential
