Qualifications

• Bachelor's or Master's degree in Human Services Administration, Psychology, Sociology, Social Work, Nursing, or Counseling.
• Valid driver’s license and a reliable vehicle.
• Preferred experience working with people with disabilities and with person-centered planning.
• Knowledge of Medicaid Home and Community-Based Services.

Responsibilities

• Advocate on behalf of people served and promote self-determination.
• Develop Person-Centered Plans of Service (POS).
• Monitor POS progress and maintain HIPAA-compliant records.
