Qualifications

• Bachelor's or Master's degree in a human service field.
• Experience with person-centered planning preferred.
• Knowledge of HIPAA and confidentiality principles.

Responsibilities

• Advocate for people served and provide resource information.
• Gather documents for eligibility and POS planning.
• Develop and update Person Centered Plans of Service.
• Coordinate and refer resources to meet POS outcomes.
• Monitor POS and ensure compliance with regulations.
• Support POS preparation and documentation with people served.
