Description

Service Coordination, Inc. is seeking Supports Planners committed to providing case management and support services for individuals with disabilities, behavioral challenges, medically complex needs, and other populations. The role focuses on empowering clients to achieve greater independence and ensuring they receive the necessary services and resources.

Position Summary

Provide case management services and support to people who have a disability and/or medical need requiring assistance with activities of daily living (bathing, dressing, toileting, eating, and mobility). Primary objectives include establishing positive relationships, exploring options and preferences, creating and monitoring Person‑Centered Plans of Service (POS), meeting state‑mandated requirements, securing services, and ensuring quality of care.

Essential Duties
  • Advocate on behalf of clients, provide resource information, and educate them to become more self‑determined.
    • Educate clients and their support system.
    • Teach coordination and advocacy strategies.
    • Encourage empowerment and optimal independence.
    • Explain principles of self‑determination.
  • Collect information and documentation for eligibility.
    • Meet with clients or authorized representatives to gather information.
    • Obtain supporting documentation.
    • Plan and update POS.
  • Plan, facilitate, develop, and update POS and related tools.
    • Engage in person‑centered planning with clients.
    • Collaborate on writing POS.
    • Prepare POS with clients.
  • Research, identify, refer, and coordinate services to meet POS outcomes.
    • Provide information, refer, and assist clients.
    • Assist with transitions.
    • Complete required paperwork and follow up for waivers.
    • Collaborate with providers for clients in institutional settings.
  • Monitor POS and conduct follow‑up activities.
    • Conduct on‑site visits and contact as needed.
    • Track progress toward goals.
    • Identify new medical and health services.
    • Complete Reportable Events (RE) per regulations.
    • Apply or re‑apply for necessary programs or services.
    • Communicate with providers regarding concerns.
    • Assist with documentation for medical assistance.
  • Adhere to SCI policies and procedures.
    • Meet performance benchmarks.
    • Maintain HIPAA records securely.
    • Use tracking and time‑management tools.

Minimum Qualifications
  • Bachelor’s or Master’s degree in a human service field: Human Services Administration, Psychology, Sociology, Social Work, Nursing, or Counseling.
Experience
  • Experience working with people with disabilities preferred.
  • Experience with person‑centered planning preferred.
Physical Demands

Standard office environment. Must be able to remain stationary >95% of the time, operate a computer and related equipment, and occasionally move office equipment weighing 10‑25 pounds. Vision requirements include close vision. Ability to operate a motor vehicle and communicate accurately with others.
