Service Coordination, Inc. (SCI) is seeking Supports Planners to serve in Maryland communities. The role emphasizes advocacy, person-centered planning, and coordinating services for individuals with disabilities or medical needs.

Eligibility requires a Bachelor's or Master's degree in psychology, sociology, social work, nursing, counseling, or human services administration. A valid driver's license and insured vehicle are required.

Qualifications

• Bachelor's or master's degree in one of the listed human services fields.
• Valid Driver's License and insured vehicle.

Responsibilities

• Advocates on behalf of the people served, provides resource information and educates them to become more self-determined.
• Collects information and documentation related to eligibility for Supports Planning services.
• Plans for and updates POS and POS tools documenting service needs for eligible people.
• Engages in person-centered planning with people served and develops POSs with them.
