Qualifications

Responsibilities

Evaluates patients and synthesizes data to establish a patient care plan with modalities, procedures, exercise programs, and education.
Gathers and interprets data from medical records, physician reports, patient, and family with documented evidence.
Sets functional goals with the patient and provides evidence-based interventions with ongoing plan adjustments.
Documents evaluations, progress notes, discharge notes, and related charges.
Communicates evaluation and progress to physicians and clinical staff; coordinates treatment schedules and discharge planning.
Supervises PTA's and Rehab Tech/Aide as required for licensure.
