Job summary

Atrium Health at Home in Charlotte seeks a licensed Doctor of Physical Therapy to manage an assigned patient caseload, deliver PT services, and coordinate care with MDs and a multidisciplinary team.

You will evaluate patients, develop and implement plans of care, document progress, and ensure compliance with state practice acts and Medicare guidelines. Travel to patients' homes is required, with supervision and opportunities for ongoing education and professional growth.

Qualifications

• DPT degree required. This aligns with current entry-level standards for physical therapy.
• License to practice physical therapy in the applicable state (e.g., North Carolina or South Carolina) is required.
• Continuing education requirements as mandated by state standards must be maintained.

Responsibilities

• Manage daily patient caseload with timely care and documentation per plan of care and MD orders.
• Collect data from home assessments and EMR to inform care decisions.
• Identify high-risk patients and coordinate case conferences to mitigate risk and ensure high-quality outcomes.
