Job summary
EMPACT-SPC, part of La Frontera, seeks a Peer Support Navigator to bridge clients to housing and behavioral health resources. The role focuses on coordinating community and RBHA-based services, facilitating access, and supporting recovery.
A background in behavioral health and personal or family experience is valued. The position is full-time with weekend/holiday hours, requiring a fingerprint clearance and a valid Arizona driver license.
Qualifications
- Minimum of High School Diploma or equivalent.
- Personal experience in behavioral health or assisting a family member.
- Peer experience, identification as the recipient of mental health services, or homeless experience in Maricopa County.
- Fingerprint clearance card or eligibility to obtain one.
- Must have a valid AZ Driver's License with clean record.
- Bilingual in Spanish preferred with differential pay.
Responsibilities
- Meets with service recipients in the community and maps housing plan steps.
- Develops network of community resources and coordinates care.
- Maintains thorough documentation in HMIS and HOME Link.
- Provides peer support and helps develop self-directed recovery plans.
- Maintains professionalism and safety in crisis situations.
