Job summary

EMPACT-SPC is seeking a Peer Support Navigator to help individuals access housing and community-based services in Maricopa County. The role emphasizes lived experience in behavioral health, recovery support, and navigating complex service systems.

The position requires engagement with RBHA resources, documentation in HMIS/HOME Link, and collaboration with clinical teams. It is a full-time role with weekend and holiday hours and preference for Spanish bilingual candidates.

Qualifications

• Minimum of High School Diploma or equivalent.
• Personal experience in behavioral health and recovery involvement.
• Identification as recipient of mental health services or experience with homelessness in Maricopa County.
• Fingerprint clearance card or eligibility to obtain one.
• Valid AZ Driver's License.
• Bilingual in Spanish is preferred (EMPACT-SPC offers Bilingual Differential Pay).
• Peer Support Certification is preferred.
• Knowledge/use of SPDAT is a plus.
• Must be 21 years of age or older.

Responsibilities

• Meets with service recipients in the community and maps out housing steps. Develops a brief housing treatment plan with individualized client goals.
• Collaborates with an extensive network of Community Based and RBHA resources, including shelters and coordinated assessment. Participates in clinical team meetings to coordinate care.
• Maintains documentation in HMIS and HOME Link.
• Provides peer support and helps develop and implement recovery plans.
• Participates in care coordination with stakeholders and service providers.
