Job summary

HOPE, Inc. is seeking a Peer Recovery Navigator to support individuals recently diagnosed with Serious Mental Illness in Tucson. You will promote recovery, provide peer support, and educate members on community resources to aid in their recovery from mental illness and addiction.

Guidance is provided by the Senior Director of Community Programs. Responsibilities include facilitating member-driven goal setting, assisting with health-home intake, and coordinating with Care Management to ensure

Qualifications

• Effective peer-support, coaching, and advocacy abilities.
• Understanding of behavioral health systems and resources.
• Strong communication and collaboration with care teams.
• Ability to engage members and family in their care decisions.

Responsibilities

• Work with newly determined SMI individuals to develop member-driven goals.
• Assist with orientation to the public behavioral health system and health-home intake.
• Model recovery, provide coaching and mentoring to develop skills toward goals.
• Educate members on their rights, and include family of choice in services.
• Assist with ISP intake into health home and advocate as needed.
• Provide check-ins after ISP is in place to ensure progress toward goals.
• Coordinate with Care Management upon identified needs.
• Ensure services are delivered to meet identified needs and outcomes.
• Facilitate communication among health care team members and members.
• Reintegrate members into the community using natural supports where possible.
• Respond to major life events and coordinate care during transitions.
• Complete timely documentation in behavioral health records.
