Job summary

Trinity Health is seeking a Peer Recovery Coach to provide recovery services for individuals at risk of substance misuse, connecting them to community resources and ensuring smooth transitions to care and treatment. You will support clients in clinics and community settings, promoting wellness and recovery through individualized coaching and group sessions.

The role emphasizes motivational interviewing, trauma-informed practice, and collaboration with a multidisciplinary team to improve outcomes

Qualifications

Experience with patients who have behavioral health conditions.
Knowledge of care transitions/Zero Suicide model preferred.

Responsibilities

Maintains good rapport and cooperative relationships.
Uses Motivational Interviewing and trauma-informed approaches to build trust.
Identifies problems, offers solutions, and participates in resolution.
Improves access through in-person interactions, screenings, and referrals coordination.
Assists patients in identifying recovery goals and navigating treatment options.
Delivers self-management skills and education to promote active participation in care.
Runs recovery groups in ambulatory and inpatient settings.
Maintains confidentiality of patient, physician, and staff information.
Completes accurate documentation in the EHR and supports care continuity.
